Lectures - Primary care and its types. Outpatient care. Occupational health services. Health assessment. Preventive care - 1 hour - Community - definition, history, social links, values and norms, supportive and self-preservation mechanisms in a community group. Community care, community nursing - 2 hours - Specification of persons who need community care - influencing factors, age, principle of care for persons with reduced self-sufficiency. Approach to persons with mental disabilities, importance of communication in helping professions and possibilities of communication with clients with difficulty communicating - 2 hours - Concept of home care, prerequisites for its provision, reimbursement decree, indications, its management, financing, quality assurance - 2 hours - Home palliative care for adult and pediatric patients, teamwork - 2 hours - Community care system for citizens with special needs - homeless people, unemployed people, people addicted to addictive substances - 1 hour Seminars - The role of the nurse in primary and preventive care. Counseling, preventive programs - 1 hour - New forms of health and social care in community-type facilities (day care centers, group housing with support). Continuity of health and social services from acute care in the hospital, through nursing and rehabilitation care, and subsequent field health and social care in the patient's/client's own social environment. Respite care - 2 hours - The role of the nurse in community care, areas of competence, indicative assessment of the patient's social situation, identification of the need for cooperation of a social or health-social worker, planning of care in cooperation with social or health-social workers. Administrative activities. Psychohygiene - 2 hours - Patient assessment and preparation for discharge (geriatric and rehabilitation conference, planned discharge, preparation of the environment, family involvement). Planning of care in the patient's own social environment. Preparation of the environment, appropriate compensatory medical devices to ensure mobility and self-care in the home environment - 1 hour - Education of patients, or other caregivers in nursing procedures, in the use of medical devices (recommendations) and preparation of information materials, Telenursing, assistive technology - 1 hour - Nursing care for patients in their own social environment. Providing psychological support. Home health care, nursing service. Activities of a nurse (specialty 925, 926), cooperation with the referring physician and other team members. Support for caregivers - 2 hour - Specifics of care for child patient and juveniles in home palliative care, care for the family and siblings of a sick child - 1 hour - Specifics of community care for people with selected specific needs (seniors, people with dementia, Alzheimer's disease, with visual and hearing impairments) - 2 hours Internship in a community care facility - 2 hours
