Lectures - Community - definition, history, social links, values and norms, supportive and self-preservation mechanisms in a community group. Community care, community nursing - 1 hour - Problems of social security and social services, orientation in social service facilities. Social policy of the Czech Republic (health policy, social security policy, social service policy) - 1 hour - Social factors affecting the admission to an acute hospital bed or after discharge to aftercare. Overall evaluation of the client and preparation for release from acute care (geriatric and rehabilitation conference, planned release, preparation of the environment, family involvement) - 1 hour - The concept of home health care, prerequisites for its provision, continuity of acute inpatient care and GP care, its management, financing, quality assurance - 2 hours - Home palliative care, hospice care - 1 hour - New forms of health and social care in community-type facilities (day care centers, group housing with support) - 1 hour Seminars - Specification of persons in need of community care - influencing factors, age, principle of care for persons with reduced self-sufficiency. Access to people with people with Alzheimer's disease - 1 hour - Resources for ensuring health and social care, possibilities of cooperation in health care at the community level, participation of the city, municipality, volunteers, self-help organizations, family members. Subsidiarity principle - 1 hour - Continuity of health and social services (acute hospital care, rehabilitation care, long-term follow-up field care, role of general practitioner, occupational therapists). Social factors affecting the admission to an acute hospital bed and after discharge to aftercare - 1 hour - The role of general practitioner, social workers, occupational therapists, rehabilitation workers, volunteers - conception of primary and community care - 1 hour - Role of nurse in community care. Specifics of community care for the elderly. Preparation of population for old age - 2 hours - The importance of communication in the helping professions, communication with clients with communication difficulties - 1 hour - Specifics of community care for people with specific needs (people with mental disabilities, with Autism Spectrum Disorder) - 2 hours - Community care system for people with special needs - seniors, homeless people and those requiring immediate help, people without employment, people with disabilities - 2 hours - Supervision as a tool for personal development of employees and prevention of burnout - 2 hours - Evaluation - 1 hour
