|
Introductory seminar for Professional Practice 2 - organizational guidelines, workplace and goals, portfolio management, logbook... (2 hrs.). Internship at a non-accredited workplace 40 hours - Operation and equipping of a hospice with medicines and medical devices, organizational and specifics of work, management of medical documentation, job description of members of a multidisciplinary team. Consultation. - Evaluation of symptoms, current personal problems and needs of a terminally ill patient/client, creation of a palliative care plan. - Evaluation of the patient's psychosocial situation; implementation of a palliative care plan with an emphasis on their quality of life, comfort and wishes. - Evaluation and monitoring of pain in the patient, interventions in the prevention of breakthrough pain. - Manipulation of an immobile patient and his positioning according to the principles of an ergonomic strategy and with an emphasis on the prevention of breakthrough pain. - Evaluation of the nature and intensity of pain and other physical symptoms and proposal of intervention. - Ordering and storing of medicines and medical devices. Documentation, records. - Cooperation in a team of healthcare professionals and other experts participating in the care of patients/clients, (pro)palliative approaches. - Communication with a terminally ill patient and loved ones, patient accompaniment procedures. - Assistance plan for family members of a terminally ill patient and psychosocial support, draft assistance plan for survivors (culture of care at the end of life). - Creation of a draft educational activities to prevent complications when providing nursing care during the absence of a nurse, according to the current status and expected course, prevention of complications of the health condition, cooperation with a loved one. - Connection/termination of patient treatment on intermittent/peritoneal dialysis. Internship at an accredited workplace 40 hours - Familiarization with the operation of a hospice, multidisciplinary team, management of health documentation, palliative care plan. - Participation in a multidisciplinary team meeting, identification of the work content of the team members - Familiarization with the equipment of the workplace with medicines and health products, including medicines and products to suppress serious symptoms of a terminal condition. - Familiarization with the procedure for ordering and storing medicines and health products, the procedure for disposing of unused opiates (in hospice-type facilities and in the natural home environment). - Familiarization with the storage of medicines requiring a reduced temperature for storage and labeling of medicines with a higher level of risk, securing addictive substances. - Familiarization with the organization and provision of palliative care in the patient's own social environment (in home hospice care). Carrying out the first visit and assessing the environment. - Participation in the creation of a palliative plan and in ensuring personal care for a patient in a terminal condition. Taking anamnesis using information from loved ones. - Identification of usual procedures for ensuring the prevention of breakthrough pain. Evaluation and continuous monitoring of pain in terminally ill patients, including patients with severe dementia (using measurement techniques). - Identification of common procedures for stimulating perception in immobile patients. - Insertion of subcutaneous cannula and application of hypodermoclysis. - Application of drug infusion subcutaneously or into a secured access (PICC, PORT, epidural catheter) to alleviate symptoms of terminal illness. - Recording of the use of a linear dispenser and recording of administered drugs in the medical documentation. Final seminar - evaluation of professional practice 2. Assessment (2 hours)
