|
- Seminar on Individual Professional Practice (introductory information, organizational guidelines, workplace and goals of practice, portfolio and logbook management) - 2 hours. Individual professional practice 40 hours - Implementation of home care, documentation management (filling out a voucher for examination/treatment in home care (DP - VZP 06). - Implementation of a nursing visit with professional performance (Type I. - IV). - Collection of anamnesis using information from loved ones, physical examination of the patient/client using assessment techniques and documentation; assessment of the patient/client's condition, comparison with the previous visit, identification of possible complications in the case of a long-term patient, creation of a care plan. - Orientation examination of mobility, assessment of self-sufficiency, self-care, hydration, nutritional status (BMI...), hydration, orientation assessment of cognitive functions and behavioral disorders in seniors using measurement techniques and scales. - Prescription of a drug selected in practice. - Skin care, assessment of skin condition and predilection sites, prevention of pressure ulcer development, assessment of the risk of patient/client neglect (including abuse). - Electrocardiogram and orientation assessment curves and assessment of the severity of the condition. - Care for a secured airway in a patient on artificial lung ventilation (humidification of the mixture, suctioning); provision of home oxygen therapy; education of the patient/client in the correct use of a personal inhaler. - Dressing of a non-healing wound using a dressing according to the healing phase; assessment and documentation of the wound; treatment of a stoma. - Catheterization of a male bladder (possibly on a model); treatment of a patient with chronic dialysis. - Treatment, dressing of vascular access. - Measurement of blood glucose levels with a glucometer and education of self-monitoring; education of the patient in the area of insulin application, prevention of diabetes complications. - Communication with service users and caregivers, including family members; communication with a patient with aphasia. - Support and counseling activities for caregivers; motivation of patients/clients, including caregivers, for education in maintaining optimal health, with the aim of achieving compliance. - Cooperation in a team of healthcare professionals and other experts in participating in the care of patients/clients. - Creating a proposal for educational activities to prevent complications when providing nursing care during the absence of a nurse, according to the current status and expected course, prevention of complications of the health condition, cooperation with a loved one; recommendations for appropriate activities to strengthen or maintain the memory, thinking and judgment of a senior. - Education of a patient/client at risk of addiction or addicted to addictive substances, counseling - possibility of participation in social programs, cooperation with a loved one. Completion of the internship - Evaluation of individual professional internship. Evaluation of the goals of the internship by a student, mentor, academic staff - 2 hours.
