Course content
Lectures - Definition of the terms home care, home palliative care for a terminally ill patient, hospice care. Concept of home care in the Czech Republic. - Organization of home and hospice care, authorization to provide health services. Conditions for accepting a patient for hospice care. Legislative issues of providing care in the patient's own social environment. - Registration forms, specialization 925 and 926 (difference and indications for acceptance into home care). Categories of providers of medical services. - Economic prerequisites for ensuring the operation of home care, personnel, material and technical equipment of the workplace, medical device rental. Financing of home care, cooperation with health insurance companies including reporting of services provided, cooperation with general practitioners, fundraising in home and hospice care. - Information systems and technologies in the patient's own social environment (electronic documentation - types, videoconferencing, assistive technologies, remote access, monitoring, safety bracelets, telenursing, risks and threats...). - Competence of specialist nurses (Nurse for home and hospice care), specialized and highly specialized nursing and palliative care activities, protection of individuals in connection with the processing of personal data, professional advice and assistance to patients and loved ones, requirements for education and further education of nurses in home care, categorization of nurses, multidisciplinary team, cooperation. - Work of a nurse in home care and hospice care. Initiation of care, admission of the patient, assessment of the health and social situation of the patient and loved ones in relation to the disease or its consequences and subsequent provision of health and social care, assessment of the patient's overall condition by a nurse, physical examination (scope), care for needs including spiritual, documentation of the patient's overall examination, scope of care provided. - Preparation of a meeting of a multidisciplinary work team in a hospice, cooperation with the family (loved one). - Specifics of home palliative care for a terminally ill patient, multidisciplinary team, support team, roles of team members including competencies. Seminars - Forms of home care. Types of visits, procedures, their planning, course, professional activities, protection and safety at work. - Material, material and technical equipment of the workplace, hygiene requirements, equipment of the visiting bag, handling of medicinal products. - Administrative activities of the nurse, keeping documentation, electronic documentation, filling out a voucher for examination/treatment in home care (VZP 06), first visit, planning of care, reporting of services provided. - Preparation of the nurse for visiting the patient in his natural social environment, general examination of the patient in home care, history taking exercises, cooperation with a loved one, communication, recording. - Specifics of nursing care for patients in the home environment with regard to age, health and psychological state. Pharmacotherapy (especially analgesia), handling of medicinal products, handling of addictive substances and preparations, documentation. - Preparation of a care plan, indications and prescription of defined medical devices for a patient in a home environment, with a view to maintaining or enhancing mobility. - Case study focusing on the issues of home, palliative and hospice care. EBN for a patient in a home environment.
